In our ongoing effort to make your pet's health care as convenient and easy as possible, you can request a refill for your pet's prescription by submitting the following form.  Please be sure to fill in all of the required information.  

Please be aware that every prescription refill must be approved by a doctor.  We can only fill prescriptions for our patients, and the patient must have been examined within the last 12 months.  For some medications, there might be lab work that is required before a refill can be provided.

We will notify you via e-mail or phone when your pet's prescription has been approved and is ready to be picked up.  We also will inform you of the total cost of the prescription and will request a credit card number by phone at that time.  If you would prefer to have the prescription mailed to you, please mention this under "additional information" on the request form.  We currenty provide FREE SHIPPING on all heartworm preventives and flea control product refills.  Shipping rates for other medications will vary.

If you would like to receive an e-mail reminder to have your pet's prescription refilled, please tell us in the "additional comments" area. There is no charge for this service.
